Physical Form – Required for Camp Attendance

General Information

1. The one-week cost for Camp Tamarack is $300.00 and includes a $40.00 non-refundable registration fee.
2. By registering your child you agree to the following: (1) the above named child has your permission to attend Camp Tamarack, affiliated with St. Albert the Great Church; (2) Camp Tamarack staff has permission to transport your child to and from camp by bus; (3) Camp Tamarack staff has permission to seek and initiate emergency medical and surgical treatment for for the above named child if needed; & (4) Camp Tamarack staff has your permission to give routine, non-surgical medical care to the above named child.
3. By registering the above named child to camp, you agree to be responsible for the entire camp fee unless a scholarship is available.
4. In Accordance with State of Minnesota law, I agree to provide witten confirmation that my child has updated immunizations and a written report of a phyiscal examination within the past 24 months.
5. Scholarships are no longer available through Hennepin County!
6. If you are interested in a partial scholarship, please contact the Davi to discuss availability. Even if you are requesting a partial scholarship, you should still send the $40.00 registration fee to hold a spot for your child.
7. In order for your child to be registered we must receive an application and the registration fee.
8. The final payment and physical form are due on July 10, 2022.
